How To: Fill Out New Hire Paperwork

Sometimes our new hire paperwork can get a little confusing, which is why Logan is here to walk you through the process step-by-step. 

① The first line on our new hire paperwork is ‘Client Name’, this is referring to the person you are working for

② Next, we just need some basic information from you like your first and last name, and your address

③ The next step to our new hire paperwork is very important. You need to fill in your social security number, and it’s crucial that you write the number as legibly as possible.

④ The next line is for your department, like ‘driver’ or ‘office’. If you don’t know your department, that’s ok, your employer can fill this out for you. 

⑤ The same applies for your pay rate, if you don’t know, your employer can fill this in for you. And if you are going to be a salaried employee, you can indicate that beneath pay rate. 

⑥ After you fill in your starting date and birth date, the next line is email address. This is also a very important piece of information, and it’s crucial that you fill it in legibly so that we can send you your pay stubs. 

⑦ Underneath this is information that can be found on your W-4. If you don’t know how to fill this section in, it’s okay to leave it blank as your employer will attach a copy of your W-4 with the new hire information. If you are a 1099 employee, you don’t need to fill this section in.

① The next form that we need you to fill out is the ‘Authorization for Direct Deposit’, the first few lines on this form are pretty straightforward: we need your name, signature, and the date.

② Then we need the name of your bank and the address. It’s important to note that we need the address of your bank’s headquarters, not just the local branch that you use.

③ Equally as important is your bank account number and routing number. these numbers also have to be written as legibly as possible so that there is no issues with readability on our end that could result in an error and you not getting paid. 

④ Please indicate whether the account is a checking or savings account. And for an added level of security, you can staple or attach a voided check with the authorization for direct deposit form.
